ScreeningRequirement for Screening
- Applicants must have a weighted average grade of 90 or better for Grade 8 and Grade 9.
- Initial qualifiers will be required to take the UPHSI SHS Entrance Examination.

Required Documents
- Grades 8 and 9 Report Cards certified as true copy by the School Principal
- Two (2) pieces 2" × 2" photo taken within the last 6 months
- Photocopy of Birth Certificate from PSA
- Photograph of the applicant in front of his/her house with full front exterior visible
- Information sheet duly accomplished and jointly signed by the student applicant and his/her parent
- Documents on family income
Important: Use and submit up-to-date admission forms. Older versions of the forms will not be accepted. Secure, attach, and review all requirements before submitting for evaluation to avoid delays.
